Thank you for choosing MPA for your child’s healthcare needs. We are delighted to work with you and are committed to providing your child with the highest quality of care. To streamline the registration process and ensure that we can schedule your first appointment promptly, please follow the detailed steps outlined below:

1. Complete the New Patient Packet:

You can fill out the New Patient Packet online by visiting the “Online Forms” tab on our website and selecting “New Patient Packet.” Alternatively, you may also print the packet from the “Policies & Forms” tab, complete it manually, and return it to either of our office locations for processing.

2. Transfer of Medical Records:

To ensure a comprehensive understanding of your child’s medical history, please complete the “Authorization of Release of Records” form, available online under the “Online Forms” tab. Once we receive and process this authorization, we will request the transfer of medical records from your previous healthcare provider. Please note that you can only schedule your first appointment after we have received all necessary medical records.

3. Appointment Scheduling:

After we have received and processed the completed New Patient Packet and the transferred medical records, our team will enter your child’s information into our Electronic Medical Records (EMR) system. Once this process is complete, we will call you to schedule your child’s first appointment with your preferred physician.
